Mantle cell lymphoma (MCL) accounts for 5–10% of mature B-cell lymphomas and is caused by the (11;14)(q13;q32) chromosomal translocation. It is an aggressive disease characterized by cyclin D1 overexpression, with cyclin D1 being an important regulator of the G1/S phase of the cell cycle. Cyclin D1 overexpression is a hallmark of MCL. However, approximately 5%-10% of MCL cases lack cyclin D1 expression, and reliance solely on cyclin D1 immunohistochemistry (IHC) testing may lead to misdiagnosis. Two reports have confirmed the expression of SOX-11 in MCL, and it shows strong nuclear expression in nearly all cyclin D1-positive MCL cases (93%-100%). In 13 cases of cyclin D1-negative MCL, SOX-11 was also strongly expressed. Additionally, researchers have identified differences between the atypical lymphoblasts of MCL and CD5+ diffuse large B-cell lymphoma (DLBCL). In summary, nuclear expression of SOX-11 is highly associated with cyclin D1 positivity and MCL negativity. This transcription factor is a useful biomarker for distinguishing true cyclin D1-negative MCL. SOX-11 IHC testing is more valuable for determining the pathological characteristics of CD5+ DLBCL. The routine use of SOX-11 in cases suspected of CD5+ DLBCL aids in distinguishing cyclin D1-negative lymphoblastic MCL. SOX-11 has also been detected in some B-cell lymphomas and T-cell lymphoblastic leukemias, although the distinct morphological and phenotypic characteristics of these malignant tumors facilitate the identification of cyclin D1-negative MCL.
